Please bring your dental insurance card, your state issued identification card and a list of any/all medications you may be taking. If you are required to take a pre-medication prior to dental visits please bring a letter from the physician requiring you to do this. If required, Just a reminder to take your pre-medication as directed by your physician for this initial appointment. Please arrive 15 minutes prior to your appointment to finalize any details with your initial registration.
